David Buchanan, MD, MS

David Buchanan leads Town Square Health as CEO, where he leads the strategic vision and execution of a patient-centered, value-based care model designed to improve access, quality, and outcomes for Medicare-eligible populations. The organization integrates primary and specialty care while prioritizing coordinated, preventive, and longitudinal care delivery.

As a board-certified internist and healthcare executive, Buchanan brings extensive experience in value-based care and clinical operations. Before establishing Town Square Health in 2025, he served as Chief Innovation Officer for CVS Healthcare Delivery, overseeing strategic improvements across Oak Street, Signify Health, and related businesses. He previously held the Chief Clinical Officer position at Oak Street Health and spent over seven years in various leadership roles. Earlier career experience included positions at community health centers, Medicaid ACOs, health IT organizations, and public hospital systems focused on health equity and reducing preventable hospitalizations.

Educationally, Buchanan earned his bachelor’s degree in Economics from MIT, his MD from the University of Chicago Pritzker School of Medicine, and completed internal medicine residency at UCSF. He holds a Master of Science degree and is a fellow of the American College of Physicians. He continues clinical practice while contributing to research and academic initiatives around health systems innovation.
