How care works at Town Square Health
Town Square Health pairs you with a primary care provider and a Care Navigator, giving patients and caregivers a consistent team to turn to as health needs change.
Primary care provider
Your primary care provider leads your care, gets to know the full picture of your health, and stays involved when additional support is needed from specialists. You can expect to have open conversations, continuity of care, and straightforward, easy-to-understand communication as part of a true partnership.
Care Navigator
Your Care Navigator is more than just your Medical Assistant. They are a team member who is with you at every visit to answer questions, coordinate services, and guide the details that can make healthcare feel hard to manage.
Specialty care support
Your primary care provider has direct access to board-certified specialists for peer-to-peer consults, often within the same day, providing specialist input without long wait times, while your PCP stays at the center of your care.
Caregiver involvement
Care often involves more than one person. At Town Square Health, caregivers are part of the conversation. They can join appointments with your primary care provider or specialist — in person or virtually — so everyone has a clear understanding of the plan and what comes next.

Primary care
Primary care services for Medicare patients are the foundation of the Town Square Health experience, helping patients stay healthier and address concerns early. This can include routine visits, screenings, wellness support, sick visits, immunizations, and other services that help maintain health.
Sick visits
When you’re not feeling well, it helps to be seen by a team that knows you. Town Square Health offers same-day or next-day sick visits for non-emergency concerns, so you can get timely care without having to start from scratch at urgent care or the ER.
Chronic condition support
For patients managing ongoing health conditions, Town Square Health provides support that helps care feel more consistent, more informed, and easier to keep up with.
Specialty coordination
When additional care is needed, we help coordinate specialty support as part of a clearer care plan. Instead of leaving patients and caregivers to manage referrals, follow-up, and communication on their own, we help bring the right people together.
Care Navigation
Healthcare can be hard to navigate. We help patients and caregivers better understand what comes next, where to turn, and how to feel more confident managing care.

Town Square Health is designed for older adults on Medicare. If you’re exploring care for yourself or someone you support, we can help you understand whether our practice is the right fit. We currently accept Aetna Medicare Advantage and will be adding more insurance plans soon.
