Dr. Buchanan founded Town Square Health with a simple belief: that patients deserve care that actually fits their lives. Before starting Town Square Health in 2025, Dr. Buchanan spent years in senior leadership roles focused on making healthcare work better for patients, including as Chief Innovation Officer at CVS Healthcare Delivery and Chief Clinical Officer at Oak Street Health, where he helped build care models centered on keeping people healthy rather than simply treating illness.
Earlier in his career, he worked within community health centers, public hospital systems, and programs serving Medicaid patients, developing approaches to care that reduce unnecessary hospitalizations and address the social and systemic factors that affect health. A Fellow of the American College of Physicians, Dr. Buchanan continues to see patients and contributes to research on how healthcare systems can do better by the people they serve.
